On your initial visit, please bring with you any requested items such as x-rays, CT or MRI scans, medications, etc. These items will greatly assist your medical provider in determining an accurate diagnosis. Our Nurse Practitioner, Kirk Douglas, will meet with you first. He will ask you about your medical history and symptoms followed by a neurological examination. A more focused neurological examination will then be performed by Dr. Daria Schooler.
We suggest that you print both of these forms, fill them out and bring them with you.
After reviewing your medical history and diagnostic tests, you will be presented with a number of treatment options. Dr. Schooler and Kirk will inform you of the risks and benefits of each option and will assist you in making the final decision concerning which option is best for you. Unfortunately, we have limited space in our waiting room and are unable to accommodate children. Please make other arrangements for them during your appointment It is our highest priority to see patients in a timely fashion however, hospital emergencies occasionally arise which make it necessary to rearrange the office schedule. Your understanding in these situations is appreciated.
